Pharmacological Category:
(1)Bacteriology:
Ampicillin/cloxacillin exhibits in vivo and in vitro bactericidal activity against Gram-positive and Gram-negative organisms. In vitro sensitivity does not necessarily imply in vivo activity.
(2)Absorption:
Both ampicillin and cloxacillin are acid stable and well absorbed orally, given peak serum levels about two hours after dosing. As there is a linear dose/response in peak serum level after oral administration of both componnents, doubling the dose virtually doubles the peak serum levels.
(3)Excretion:
Both components are excreted primarily by the kidneys by glomerular filteration and tubular secretion. Bile concentrations of ampiillin/cloxacillin vary from 3-48 times the serum concentration, according to the condition of the biliary tract.
(4)Probenicid:
Higher ampicillin /cloxacillin serum levels can be achieved in patients with normal renal function by the concurrent administration of a renal blocking agent such as probenicid. In adilts, a dose 500mg of probenicid four times daily will result in 1.5-2 fold increase in ampicillin/cloxacillin serum level.

Indications:
Infections cause by susceptible organisms where a mixed infection is present and includes penicillin-resistant staphylococci. Useful in the treatment of respiratory tract infection, ear, nose, and throat infections, urinary tract infections, gastro-intestinal infections, skin and soft tissue infections, pelvic infections, septicemia, endocarditis, orthopaedic infections.

Contraindications:
Penicillin hypersensitivity, ocular administration-this must not be used in the eye either sub-conjunctivally or locally.

Warnings:
Before initiating therapycareful inquiry should be made concerning previous hypersensitivity reactions to beta-lacams. Anaphylactic shock may occur.  corticosteroids and anti-histamines should be used to treat anaphylaxis.

Precautions:
Dosage should be adjusted in patients with renal impairment.

Side Effects:
As with other penicillin drugs, side effects are usually of a mild and transitory nature.They may include nausea, diarrhea, indigestion, or occasionally rash, either urticarial, which suggests penicillin hypersensitivity, or erythematous. An erythematous rash may occur in patients with glandular fever. In either case it is advisable to discontinue therap. As with other beta-lactam antiviotics, anaphylaxis and other hypersensiticity reactions (including interstitial nephritis) have been reported rarely. Bullous reactions have been reported very rarely. Other side-effects include jaindice or the transient increase in transaminases. Ampicillin/cloxacillin may decrease the efficacy of oestrogen containing oral contraceptives
