Indications:
Long-term treatment of coronary heart disease; prevention of angina pectoris; treatment of persistent angina pectoris after myocardial infarction; combined application with digitalis and/or diuretics to treat chronic congestive heart failure.

Dosage:
Dilute with 5% glucose injection and start intravenous infusion from 1 to 2 mg/hr. Adjust the dose according to the patient's response. The maximum dose is 8 to 10 mg. During the medication, the patient's heart rate and blood pressure must be closely observed. Due to the different individual responses, the dose needs to be adjusted individually.

Adverse reactions:
Vasodilatation headaches caused by nitrate esters may appear at the beginning of the medication, and usually after several days of continuous use, the symptoms disappear. Facial flushing, dizziness, orthostatic hypotension, and reflex tachycardia may also occur. Occasionally, blood pressure decreased significantly, bradycardia, aggravated angina pectoris, and syncope.

Taboo:
Acute circulatory failure (shock, circulatory collapse); severe hypotension (systolic blood pressure <90 mmHg); acute myocardial infarction with low filling pressure (unless under conditions of continuous hemodynamic monitoring); hypertrophic obstructive cardiomyopathy ; Constrictive pericarditis or pericardial tamponade; severe anemia; glaucoma; increased intracranial pressure; allergic to nitro compounds.

Pharmacology:
Isosorbide mononitrate (ISMN) is the main biologically active metabolite of isosorbide dinitrate. Like other organic nitrates, its main pharmacological effect is to relax vascular smooth muscle. ISMN releases nitric oxide (NO), which is the same as endothelial relaxing factor, activates guanylate cyclase, increases cyclic guanylate (cGMP) in smooth muscle cells, relaxes vascular smooth muscle, and expands peripheral arteries and veins. It has a stronger effect on vein expansion. The venous dilation causes blood to be retained in the periphery, the return blood volume is reduced, and the left ventricular end diastolic pressure and pulmonary capillary wedge pressure (preload) are reduced. Arterial expansion reduces peripheral vascular resistance, systolic arterial pressure and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris.
