Mannitol Injection 20%
Name: Mannitol Injection
Main ingredients: D-Mannitol
Structural formula : C6H14O6
Mol wt: 182.17
Description: The product is clear colorless liquid.
Indication:
1. Tissue dehydrator. It is used to treat brain edema caused by various reasons, reduce intracranial pressure and prevent brain hernia.
2. Reduce intraocular pressure. It can effectively reduce intraocular pressure, and can be used in preparation before intraocular surgery when other intraocular pressure lowering drugs are ineffective.
3. Osmotic diuretics. It can be used to identify prerenal factors or oliguria caused by acute renal failure. It can also be used to prevent acute tubular necrosis caused by various reasons.
4. As an auxiliary diuretic measure to treat nephrotic syndrome, cirrhosis ascites, especially when accompanied by hypoproteinemia.
5. It can promote the excretion of the above substances and prevent nephrotoxicity in case of overdose of some drugs or toxicosis (such as barbiturates, lithium, salicylate and bromide, etc.).
6. As irrigating agent, it is used in transurethral prostatectomy.
7. Preoperative bowel preparation.

contraindications:
1. Anuria patients diagnosed as acute tubular necrosis, including those who did not respond to mannitol, increased blood volume due to mannitol accumulation and increased heart burden;
2. Severe water loss;
3. Patients with active intracranial hemorrhage, except for intracranial surgery, were aggravated by volume expansion;
4. Acute pulmonary edema or severe pulmonary blood stasis.
side effect:
1. Disturbance of water and electrolyte is the most common.
(1) Rapid intravenous injection of mannitol can cause the accumulation of mannitol in the body, increase the blood volume rapidly (especially in acute and chronic renal failure), lead to heart failure (especially in the case of heart function damage), dilute hyponatremia, and even hyperkalemia;
(2) Inappropriate excessive diuresis leads to decrease of blood volume and aggravation of oliguria;
(3) A large amount of intracellular fluid transferred to the outside of cells can cause dehydration of tissues and symptoms of central nervous system.
2. Shivering and fever.
3. Dysuria.
4. Thrombophlebitis.
5. Mannitol extravasation can cause tissue edema and skin necrosis.
6. Allergy causes rash, urticaria, dyspnea and anaphylactic shock.
7. Dizziness and blurred vision.
8. Hyperosmolarity causes thirst.
9. Osmotic nephropathy (or mannitol nephropathy) is mainly seen in high-dose rapid intravenous drip. The mechanism has not been fully elucidated, which may be related to mannitol induced renal tubular fluid osmotic pressure rise too high, leading to renal tubular epithelial cell damage. The pathological manifestations were swelling of renal tubular epithelial cells and formation of vacuoles. Clinically, there is a decrease in urine volume, and even acute renal failure. Osmotic nephropathy is common in elderly patients with decreased renal blood flow, low sodium and dehydration.
