Indication:
1. This product is used as the first choice or drug for the following diseases.
(1) Rickettsial diseases include epidemic typhus, endemic typhus, Rocky Mountain fever, tsutsugamushi disease and Q fever.
(2) Mycoplasma infection.
(3) Chlamydia infection, including psittacosis, venereal disease, lymphadenopathy, nonspecific urethritis, salpingitis, cervicitis and trachoma.
(4) Regression fever.
(5) Brucellosis.
(6) Cholera.
(7) Rabbit fever.
(8) Plague.
(9) Chancre.
The treatment of brucellosis and plague should be combined with aminoglycosides.
2. Due to the serious resistance to tetracyclines of common pathogenic bacteria at present, it is only when the pathogenic bacteria are sensitive to the drug that the drug should be used. This product is not suitable for the treatment of hemolytic streptococcal infection and any type of staphylococcal infection.
3. This product can be used in patients with penicillin allergic tetanus, gas gangrene, Yashi, syphilis, gonorrhea and leptospirosis, as well as actinomycetes and Listeria monocytogenes infection.
Adverse reactions:
1. Gastrointestinal symptoms such as nausea, vomiting, epigastric discomfort, abdominal distension, diarrhea, etc., occasionally lead to pancreatitis, occasionally report of esophagitis and esophageal ulcer, mostly occur in patients who are immediately bedridden after taking medicine.
2. This product can cause hepatotoxicity, usually fatty liver degeneration. Pregnant women and patients with original renal damage are prone to hepatotoxicity, but hepatotoxicity can also occur in patients without the above conditions.
3. Allergic reactions: most of them are macular papules and erythema. Urticaria, angioneurotic edema, Henoch Schonlein purpura, pericarditis and systemic lupus erythematosus can be seen in a few patients. Exfoliative dermatitis is not common. Anaphylactic shock and asthma occurred occasionally. Some patients with tetracycline will be photosensitive when exposed to the sun. Therefore, it is suggested that patients should not be directly exposed to sunlight or ultraviolet rays during taking this product, and stop taking the drug immediately once the skin has erythema.
4. Blood system: it can cause hemolytic anemia, thrombocytopenia, neutropenia and eosinophilia occasionally.
5. Central nervous system: it can occasionally cause benign intracranial pressure increase, which can be manifested as headache, vomiting, optic nerve papilla edema, etc.
6. Nephrotoxicity: Patients with original significant renal impairment may have aggravation of azotemia, hyperphosphatemia and acidosis.
7. Double infection: long term use of this product can cause digestive tract, respiratory tract and urinary tract infections caused by drug-resistant staphylococcus aureus, gram-negative bacilli and fungi, and even sepsis in severe cases.
8. The application of tetracyclines can reduce the normal flora in human body, resulting in vitamin deficiency, fungal reproduction, dry mouth, pharyngitis, angular stomatitis, glossitis, dark or discolored tongue coating, etc.
Matters needing attention:
1. Cross allergic reaction: those who are allergic to one tetracycline are allergic to other tetracyclines.
2. Interference to diagnosis
(1) Due to the interference of tetracycline on fluorescence, the determination result of urine catecholamine (hingerty method) is higher.
(2) This product can increase the determination values of alkaline phosphatase, blood urea nitrogen, serum amylase, serum bilirubin and serum aminotransferase (AST, ALT).
3. Regular follow-up should be conducted to check blood routine and liver and kidney function during long-term medication.
4. When using this product, you should drink enough water (about 240ml) to avoid esophageal ulcer and reduce gastrointestinal irritation.
5. This product should be taken orally on an empty stomach, i.e. 1 hour before meal or 2 hours after meal, to avoid the influence of food on absorption.
6. When the following situations exist, it should be used with caution or avoided.
(1) As this product can cause liver damage, it is not suitable for patients with original liver disease.
(2) As this product can aggravate azotemia, it is not suitable to use this kind of drug because of renal function damage. If there are indications, it should be carefully considered and reduced according to the degree of renal function damage.
7. In the treatment of sexually transmitted diseases, if spirochete infection is suspected at the same time, dark field microscopy and serological examination should be performed before medication. The latter should be performed once a month, at least four times.
Drug interactions:
1. When used together with antacids such as sodium bicarbonate, the absorption and activity of the product will decrease due to the increase of pH value in the stomach. Therefore, antacids should not be taken within 1-3 hours after taking the product.
2. The drug containing calcium, magnesium, iron and other metal ions can form insoluble complex with this product, which can reduce the absorption of this product.
3. When combined with general anesthetics methoxyflurane, it can enhance its nephrotoxicity.
4. When combined with strong diuretics such as furosemide, it can aggravate the damage of renal function.
5. When combined with other hepatotoxic drugs (such as anti-tumor chemotherapy drugs), it can aggravate the liver damage.
6. The absorption of the product can be affected by the antihyperlipidemic drugs, which must be taken separately several hours apart.
7. This product can reduce the effect of contraceptives and increase the possibility of extramenstrual bleeding.
8. This product can inhibit the activity of plasma prothrombin, so patients receiving anticoagulant therapy need to adjust the dosage of anticoagulant.
