indication:
Penicillin V potassium is suitable for mild and moderate infections caused by penicillin sensitive strains, including tonsillitis, laryngitis, scarlet fever, erysipelas caused by streptococcus; bronchitis, pneumonia, otitis media, sinusitis caused by pneumococcus; and skin and soft tissue infections caused by sensitive Staphylococcus. Penicillin V potassium can also be used for the prevention of rheumatic fever recurrence and infective endocarditis, and can also be used for spirochete infection.
Usage and dosage:
Oral administration. Adult: Streptococcus infection once 250 mg (1 capsule), 3-4 times a day, 10 days of treatment. Pneumococcal infection once 250 ~ 500 mg (1 ~ 2 capsules), 4 times a day, treatment until fever subsided at least 2 days. Staphylococcus infection, spirochete infection (fenson's angina), 250 ~ 500mg (1 ~ 2 capsules), 3 ~ 4 times a day. To prevent recurrence of rheumatic fever, 250 mg (1 capsule) twice a day. To prevent endocarditis, take 2g (8 capsules) orally one hour before tooth extraction or upper respiratory tract surgery, and then take 1g (4 capsules) six hours later (half the dose for children under 27kg).
Adverse reactions were as follows
The common adverse reactions of oral penicillin V were nausea, vomiting, epigastric discomfort, diarrhea and black tongue. Allergic reactions included rash, urticaria and other serum disease like reactions, laryngeal edema, drug fever and eosinophilia. Hemolytic anemia, leukopenia, thrombocytopenia, transient elevation of serum transammonium, neurotoxicity and nephrotoxicity were rare. Long term or large-scale use of this product can cause double infection of penicillin resistant Staphylococcus aureus, gram-negative bacilli or Candida albicans.
Taboo:
It is forbidden for patients with infectious mononucleosis. It is forbidden for those who have allergic history to penicillin drugs and those who have positive reaction to penicillin skin test.
matters needing attention:
(1) Penicillin skin test must be done before using this product. The concentration of skin test solution is 500 units per 1ml. Intradermal injection of 0.1ml is forbidden for positive reaction.
(2) Cross allergic reaction: a person allergic to one penicillin may be allergic to other penicillins, or to penicillamine or cephalosporin. Use with caution for those allergic to cephalosporins. In case of allergic reaction, stop using the product immediately and take corresponding measures.
(3) The following conditions should be used with caution:
1.patients with allergic diseases such as asthma, eczema, hay fever and urticaria;
2.the elderly may need to adjust the dose;
3.patients with renal dysfunction should adjust the dose or the interval of administration according to the plasma creatinine clearance rate.
(4) Interference on diagnosis:
1.during the application of this product, false positive reaction may appear when urine glucose is determined by copper sulfate method, but the glucose enzyme method is not affected;
2.the application of most penicillins can increase serum alanine aminotransferase or aspartate aminotransferase.
(5) Patients who take this product for a long time or a large dose should regularly check the function of liver, kidney and hematopoietic system, and test serum potassium or sodium.
(6) The course of treatment for streptococcal infection should be 10 days, and bacterial culture should be performed after the treatment to determine whether the Streptococcus has been eliminated.
(7) For suspected gonorrhea patients with syphilis damage, dark field examination should be performed before using this product, and serum test should be conducted once a month for at least 4 months
